You need to submit a designation form of tax agent to the tax office where you reside last
After you leave Japan you need to designate a Tax Agent (Nozei Kanrinin, 納税管理人 in Japanese) in order to claim your tax refund in Japan. Your tax agent will claim your tax refund on behalf of yourself.
To do so you need to fill and submit a designation form of Tax Agent (Nozei Kanrinin Sennin Todoke, 納税管理人選任届出書) with information of yourself as well as Tax Agent you designate.

The tax office (Zeimusho, 税務署) where you reside last is the office you can submit it. You can confirm your last residence on your Residence Card (Zairyu Card, 在留カード). You can find the tax office of your residence in National Tax Agency’s website, shown below. Please fill Japanese Zip Code (Yubin Bango, 郵便番号) on it.

You need to submit two forms in order to replace old Tax Agent for new one
If you need to replace your Tax Agent for a new one, then please fill two kinds of forms and submit them to the same tax office:
- (1) Remove form of Tax Agent (Nozei Kanrinin Kainin Todoke, 納税管理人解任届出書). Please fill information of yourself as well as Tax Agent you remove.
- (2) Designation form of Tax Agent (Nozei Kanrinin Sennin Todoke, 納税管理人選任届出書). Please also fill information of yourself as well as Tax Agent you designate anew.
